Bile Duct Cancer & Diagnosis

Bile Duct Cancer & Diagnosis

Bile Duct Cancer:

Bile Duct Cancer, or Cholangiocarcinoma the name of the disease in medical language a rare, but very aggressive kind of cancer. A greenish fluid secreted in the liver and stored in the gall bladder. Bile enormously essential to aid the digestion system and absorption in the small intestine. The bile Duct is an important organ of the human body. Performs a vital function in the human body. a duct-like tube that sends Bile (fluid) from the Liver and Gall Bladder to the duodenum (first section of the small intestine).

Malignant (cancer) cells are developed in the Bile Duct in this disease. The risk of cancer can increase due to certain liver diseases. Pain in the abdomen and jaundice can symptoms of Bile Duct Cancer. Weight loss and yellowish skin when Bile Ducts blocked by a tumor. Generalized itching, fever dark urine, and colored stool can also the symptoms of Bile Duct Cancer.

Diagnosis of bile duct cancer:

There are specific tests to detect, diagnose, and know the stage of Bile Duct and nearby organs’ cancer. For the diagnosis of bile duct cancer sample of tissues obtained through different procedures. Including Blood chemistry tests, the Tumor marker test, Biopsy, Ultrasound Computed Laparoscopy, Tomography (CT or CAT) scan, Magnetic resonance imaging (MRI), and PTC (Percutaneous Transhepatic Cholangiography.

Anything that affects the chances of getting cancer or any other disease is called a risk factor. Some research studies suggest about 40% of cholangiocarcinoma (Bile Duct Cancer) patients are victims of identifiable risk factors. whereas factors like aging, genetic predisposition cannot be altered.

Exact causes are still not known. But some research studies suggest some possible risk factors including Bile Duct stones, invasion, parasite, or inflammation.

The parasitic disease can also a risk factor. A parasitoid an infection caused or transmitted by parasites. Other causing factors may Alcoholic liver disease, parasitic infections, Human immunodeficiency (HIV) infection, Bile-duct cysts, Liver Cirrhosis, Hepatitis C virus infection, Diabetes, Inflammatory bowel disease.

-More about diagnosis:

Some measures are taken to minimize the risk factors such as limiting the use of alcohol. taking a healthy diet, not smoking, physical active ties, and vaccination against the hepatitis B virus. Patients with early-stage diagnosed Bile Duct cancer have a 5-years survival ratio estimated at 30%. The 5-years survival rate is 24% if cancer spread in nearby regional lymph.

5-years survival ration just 2% if cancer spread to a distant part of the body. Even after transplant surgery,  the survival rate typically 50%. Cholangiocarcinoma ratio is nominal in the Western world with a ratio of 0.2 persons in 100, 000 annually.

The ration of Bile Duct cancer higher in East Asian countries and especially Thailand. Where the disease of Live Flukes common and about 60 persons in 100, 000 the cases of this cancer annually.

The rate of this disease at the rise in even the Western world. About 2500 new cases of Bile Duct cancer are diagnosed annually in the US alone.

Itching (about 66%), abdominal pain (30 to 50%), weight loss ( 30 to 50%), fever ( in about 20%), and darkness in stool and urine color its symptoms.

The procedures generally conducted for the diagnosis are as follows:
  • ERCP procedures.
  • Endoscopic retrograde cholangiopancreatography (ERCP).
  • Endoscopic ultrasound.
  • Biopsy: A procedure in which getting a small sample of tissue for examination through a microscope is called a biopsy.
  • LFT: Liver Function Test.
  • Images tests. Through these tests, doctors try to examine the abnormalities in the internal organs that may show Bile Duct cancer.
  • CT scans Computerized Tomography scans.
  • The Magnetic Resonance Imaging (MRI) combined with.
  • Magnetic Resonance Cholangiopancreatography (MRCP).

Cholangiocarcinoma one of the most difficult types of tumors to treat. don’t hesitate to ask about your doctor’s experience with treating the condition.

Bile duct cancer’s or Cholangiocarcinoma treatments may include:
  • Surgery ( removing cancer affected portion of . if possible).
  • Liver transplant.
  • Chemotherapy ( killing the cancer cells y using specific medicines).
  • Radiation therapy (Destroying cancer cells by using high-energy rays, X-rays, photons).
  • Photodynamic therapy.
  • Biliary drainage (A procedure to restore the flow of Bile through bypass surgery around cancer affected area or by stents).
  • Targeted Therapy Drugs.
  • Immunotherapy.
  • Palliative Therapy.

By adopting a healthy pattern of life, exercises, natural and organic healthy food. We can minimize the chances of various diseases including cancer. The use of insecticides, pesticides, herbicides, and other poisonous medicines in crops and other food and yields increases the risks of fatal diseases also including various types of cancers. The more we adopt natural ways of life and organic foods the more we develop immunity against diseases.

LIVER CANCER IN CHILDREN

LIVER CANCER IN CHILDREN

LIVER CANCER IN CHILDREN:

The Liver’s largest organ found in the human body. It involved in many important functions and chemical reactions and considered as the metabolic factory of our body. It stores carbohydrates, uses fats to provide energy, stores minerals, and vitamins. Breaks down proteins, and also involved in the production of blood clotting factors.

Cancer! This six-letter word can make your whole existence upside down. one of those diseases that spread faster than anything else in the entire world. A very invasive and destructive enemy of our body. The body cells start to grow and divide uncontrollably. And then they migrate to different sites of the body. which can even result in death if not cured on time. Cancer different types. Here we will focus on liver cancer in Children.

Symptoms of liver cancer in Children

Liver cancer in children known as Hepatoblastoma. It occurs in young children in the form of a tumor in the liver. The cause of Hepatoblastoma still not defined, no genetic linkage but it usually occurs to those children who develop the condition of Beckwith-Wiedemann or have a family history of familial polyposis coli. It occurs between the ages of one and two years.

The symptoms of liver cancer, include pain in the abdomen although the symptoms become more prominent when the size of the tumor gets enlarged. There a loss of appetite, swollen belly, nausea and vomiting, lethargy and fever, etc.

The first step of investigation involves tests to diagnose the disease in order to determine the level of cancer and then to treat it accordingly. The diagnoses involve blood tests to detect the presence of tumors in the blood. Then the ultrasound of the liver done in order to find out the size of the tumor, and then a detailed CT scan of the abdomen to check if it spread. Then finally biopsy of the tumor done to make the final diagnosis.

chances of recovery and treatment:

The chances of recovery and treatment depend upon the stage of cancer. The size of the tumor, relapse, or the risk group your child falls into.  The treatment usually involves surgery, chemotherapy, and liver transplantation if needed.

Chemotherapy carried out in order to destroy the cancer cells and to shrink them. It is given through injections in the vein. Some side effects of chemotherapy such as hair loss, nausea and vomiting, bone marrow suppression, and sore mouth.  After chemotherapy, surgery performed by a liver surgeon to completely remove the tumors from the area.

Liver Cancer & Its Causes

Liver Cancer & Its Causes

In the past, people thought that only hepatitis and only severe liver conditions would lead to liver cancer. But that’s not the case anymore. According to the experts. it is even threatening for those who have Diabetes, Obesity or are consuming unhealthy foods. Cancer is a deadly disease that can silently attack, anyone and you would not even know for a long time.

Our lifestyle plays an important role in how long we live. We think that cancer wouldn’t happen to us because we do not have any liver diseases. but it can still happen to you if you are overeating. And too only unhealthy food without working out or exercising.

Research suggests that exercising every day reduces the risk of liver cancer or any type of cancer in your body. Working out for 20 minutes daily can make your body very active and healthy. A study was conducted in Italy where people who ate more fruits and vegetables found a 50 percent reduction in liver cancer. even if they were suffering from other liver diseases. Consuming steroids and hormones can also cause liver cancer. because they damage our liver heavily.

More about:

There are some symptoms such as abdominal pain. heaviness in the chest, nausea, and loss of appetite, etc. if you have such symptoms then you must get yourself checked. Even if you do not have any symptoms you should get yourself screened every year. it is good for your peace of mind and future.

Liver Transplantation, A chance to life

Liver Transplantation, A chance to life

Liver Transplantation:

Liver transplantation is the blessing that has been saving many lives! It is one of the products of advanced medical technologies and techniques. This procedure is usually done on the patients suffering from cirrhosis which happens because of consumption of alcohol or hepatitis C and then the damaged liver is replaced with another liver donated by a deceased or a living donor. The surgery takes its time according to the condition but usually, it lasts from 4 to 14 hours.

Mainly, there are three types of liver transplants. The most common one was Orthotopic liver transplant where the liver is taken out from a dead or deceased donor. Living donor transplantation requires a living donor who is willing to donate their liver to the patient.

Liver transplantation is a ray of hope for the patients suffering from severe liver-related diseases because the liver is the only organ that has the property to regrow to its full size after the surgery. There are some factors that have to match or be compatible with the transplant to be performed. It’s not in the control of anyone; it depends on your luck or how fast you can find the compatible donor!

Changes after liver transplantation:

Certain lifestyle changes are required for a liver transplant. A transplanted liver becomes sensitive to chemicals such as alcohol, sprays, and toxins. The recipients of liver transplant should avoid coming in contact with fumes and drinks as such. Drinking a lot of water is necessary! But it should be boiled or chlorinated so that it doesn’t harm your body. Of course, there will be a scar on your body post-transplant but that’s alright. If you face such a big procedure than you can face that too!

Dr. Faisal Saud Dar is a blessing for us Pakistanis as this procedure in the past was only performed in foreign countries and coasted a lot of money. But now! We have this facility in our very own country Pakistan.

How Hepatitis C Affects Human Liver

How Hepatitis C Affects Human Liver

Hepatitis C is a chronic viral inflammation of the liver that can cause temporary or permanent liver damage. The liver damage at first in the form of liver inflammation gradually leading to scarring or fibrosis Hepatitis C Affects Human Liver.

Hepatitis C can cause the death of liver cells. This might be due to either virus-killing liver cells or immune system response to infected liver cells. Recent research shows that its combination of both factors but immune system response seems more pronounced causing much damage. The death of liver cells causes inflammatory changes in the human liver. That in turn causes enlargement of the liver (hepatomegaly). Liver enlargement seen in about 60% of patients and the leading cause of persistent pain in the liver area.

Inflammation triggers:

Inflammation triggers the processes that lead to fibrosis. Fibrosis is not a disease but is a condition caused by the body’s response to liver damage. Specific liver cells called stellate cells are triggered by Inflammation. These cells store fat and vitamin A when the liver is in a healthy condition. These cells are also responsible for blood flow regulation within the liver Hepatitis C Affects Human Liver. But when the liver inflamed by the presence of hepatitis C, a reaction occurs amongst different liver cells. This reaction leads stellate cells to dispense with vitamin A, altering their function.

Infected and inflamed liver cells:

Infected and inflamed liver cells release chemical signals called ‘cytokines’. These chemicals cause the activation of white blood cells outside the liver. The activated white blood cells travel to the liver and there in combination with specialized white cells of the liver, produce further chemical signals. These signals cause stellate cells to begin producing and laying down collagen fibers. In the extracellular matrix, which is the area between the cells.

Collagen, a fibrous protein:

Collagen, a fibrous protein is fundamental to the formation of scar tissue. The human body has the protective mechanism of limiting infection and healing of injured areas. For this purpose it uses collagen. In normal conditions when the particular injured area is healed or infection resolved the collagen is dissolved and removed from that area. The activated stellate cells then die off, allowing the tissue to return to normal. But in chronic hepatitis C, the collagen matrix grows too fast and cannot be properly dissolved. Resulting in scar tissue build up around the cells. This prevents liver cells from getting vital nutrients and oxygen from blood so they die rapidly. A never-ending cycle starts in which inflammation and fibrogenic cells stimulate each other leading to increased fibrosis.

Free Radicals and Fibrosis:

A further possible cause of fibrosis due to damage by free radicals. Free radicals are highly reactive chemical substances. They are the by-product of a cell’s normal reactions such as energy generation and the breakdown of fats. During these reactions, oxygen is transformed into the free radical superoxide. Cells with normal physiological function protection mechanisms to protect themselves from damage. Effects of free radicals but if free radicals in high concentration or cells have lost normal function. Then free radicals can cause havoc Hepatitis C Affects Human Liver.

Free radicals much concerned with hepatitis C patients due to the following reasons.
  • Chronic liver inflammation may lead to over-production of free radicals within the liver.
  • There is evidence that free radicals play a role in liver fibrosis. Free radicals can chemically alter fat in the body. This called lipid peroxidation. The free radicals attack the cell membrane and can injure and eventually kill cells. If this happens to liver cells, it will lead to fibrosis.
  • When the liver function is already impaired and there an overload of iron, the free radicals may interact with the iron causing further damage.

The liver tissues wonderful potential to regenerate, so why they do not regenerate in Hepatitis C infection.

Hepatitis C causes slow tissue damage and progressing scarring. Our liver two protective mechanisms against any damaging factor. Either their regeneration with complete restoration of the liver structure and function or there sustained scarring of liver tissue leading to damage. When the liver damaged by a single strong injury such as in donating a part of the liver. Regeneration highly likely even if a large area affected. But if the injury repetitive and the liver does not have enough time for healing from a previous injury and next injury ensues. As the case with hepatitis C infection – the liver cannot effectively cope. It does not the time and space to heal and regenerate.

Hepatitis C in Pakistan

Pakistan is enduring an HCV epidemic of historical proportions—one in every 20 Pakistanis already infected. With this infection playing a major role in liver disease burden in this country. HCV prevalence high in all risk populations with most transmission apparently driven by healthcare procedures Hepatitis C Affects Human Liver. Malpractice, misdiagnosis, use of nonsterile surgical and dental equipment, and using a single syringe more than once all are largely contributing factors. Drug abusers, sex workers, and malnourished children are high-risk groups.

More about Hepatitis C:

We strive for community awareness against Hepatitis C and urge the government to make HCV treatment and prevention of national priority in Pakistan. Although the country has made efforts to increase coverage of safe injection and blood screening and to improve infection control commitment to prevention in all segments of the healthcare system, including the private sector, still the target of the complete elimination of hepatitis C seems very distant.
